How travel pillows actually work
That single detail accounts for most of the reviews saying these things are useless. Below: the mechanics, what the shape is and is not built to do, and how to tell within one flight whether yours is doing anything.
What the shape is actually for
Sitting upright, your head weighs around 11 lb and nothing is carrying it. Lying down, a mattress takes that weight and your neck muscles stand down. In a seat, they stay on duty.
When you doze off they let go, the head starts to fall, and they snatch it back — and you surface. That cycle repeats every few minutes for the whole flight, which is why people describe seven hours of not-quite-sleeping rather than seven hours of no sleep.
A neck pillow's entire job is to give the head somewhere to arrive that is not the end of a muscle. Get that right and the cycle stops. Get it wrong and the pillow is decoration.
Why so many people conclude they do not work
Four reasons, in the order they come up:
- Worn back to front. The gap belongs under your chin. Your head does not fall backwards — the seat is behind you. It falls forwards and sideways, and the gap at the back is doing nothing about either.
- Too loose. If there is a hand's width between the two ends under your chin, the head still has room to accelerate before it meets anything. The ends should nearly touch.
- Bead-filled or inflatable. Beads move out from under the load exactly like a fiber pillow does; an inflatable one is a balloon that changes shape with cabin pressure. Neither holds a position for hours.
- Sitting bolt upright. With no recline your center of mass is in front of your base, and no amount of neck support fixes a torso that wants to fold forward.

What a travel pillow is responsible for
Worth being precise, because the promise gets inflated and then disappoints.
It holds your head still while your neck muscles are off duty. That is the claim, it is mechanical, and you can feel whether it is true within about twenty minutes of closing your eyes.
What decides everything else on a flight is the seat, the light, the noise and the timing. A pillow that is doing its job on a bolt-upright middle seat next to the galley at midday is still a pillow on a bad flight. Those factors are handled in sleeping on a plane and choosing a seat, and they are worth more than most people expect.
Memory foam versus the alternatives

The property that matters is the same one that matters in a bed: does it still hold its shape at hour four? That is what separates the pillows people keep from the pillows people leave in a seat pocket.
The twenty-minute test
You can settle the question on your next flight without buying anything.
Put the pillow on with the opening at the front, close the gap under your chin, sit back, recline what you can, and shut your eyes for twenty minutes. Then notice one thing: did your head move?
If you surfaced two or three times because your chin dropped, something in the four reasons above is still wrong. If you opened your eyes and your head was roughly where you left it, the pillow is working — whatever you managed to sleep, the mechanism is doing what it is for.
Is it worth getting one?
If you fly or travel long distances more than a couple of times a year, yes — on the arithmetic alone. A flight arrived at rested is worth a good deal more than the cost of the object, and the difference between arriving functional and arriving wrecked is often just whether your head stayed still.
If you travel rarely and only on short hops, you will get less out of it, because the mechanism needs a stretch of time to matter.
Either way, the thing that decides it is not which one you buy — it is whether you wear it the right way round. That costs nothing and it is where most of the disappointment in this category comes from.
